“We are living in an era of home delivery”, says Val J. Halamandaris, President of the National Association for Home Care & Hospice (NAHC). “Home care services can provide just about anything you can think of”.

And that’s part of what makes home health care so appealing. These days, home health care brings a variety of health and social services right to your door.

Types of home health care services include:

  • Physical, speech and occupational therapy
  • Pain Management
  • Medication Management
  • Wound Care
  • Infusion Services
  • Diabetic Care
  • Assistance with cooking and other household duties, and with personal needs, such as bathing
  • End of life care/comfort care

Home Health Care provides nurses, physical and occupational therapists, social workers, and home health aides to provide skilled care in the home.

Because Home Health Care provides such a broad range of services, it benefits many types of people, including adults with disabilities, people recovering from illness or injury, and people who are chronically or terminally ill.

Choosing a Provider.

The first step in finding a Home Health Care provider is to determine the services you need. Next, ask your doctor to order services for you. How to qualify for Home Health services?

  1. You must live in our geographical service district. This includes the towns of Astoria, Gearhart, Knappa, Warrenton, Hammond, Westport and Seaside to the Highway 101/26 junction.
  2. You must have the need for skilled services requiring the care of a Registered Nurse, Physical Therapist, Speech Therapist, or Occupational Therapist.
  3. Personal hygiene, bathing, simple meal preparation, or light laundry services may be provided by a Home Health Aide but only if skilled services are being provided and insurance authorizes payment.
  4. Your physician must order the services.
  5. You must be home bound. That is, your condition restricts activities so that you are not routinely able to leave the home except for medical purposes or occasional outings that are infrequent and of short duration, and require the assistance of people or devices to leave the home. Additionally, these activities must be taxing and difficult.
  6. You may not drive a motor vehicle.

It is important that your provider know what medications you are on. Please click here and download a copy of our Medication Information Form. Fill it out and bring a copy with you when visiting any healthcare provider. It is a good idea to make copies of the form and carry one with you at all times.
