Hospice House
The CMH Hospice House is a licensed adult foster home in Warrenton Oregon, providing residential care for hospice patients.
The Hospice House is a licensed adult foster home located on the east side of Smith Lake in Warrenton. The house sets on a large lot looking west toward tall trees and the lake. A large deck provides outside enjoyment. Birds and water fowl abound and the ocean can be heard in the distance.
24 hour supervised care is provided. All residents are enrolled as clients of Lower Columbia Hospice and have interdisciplinary care planned and implemented by employees of Columbia Memorial Hospital. Each resident has an assigned RN who supervises all nursing care. Other services provided include all therapies as needed or requested by a physician, a Medical Social Worker, Home Health Aide and Chaplain services. Your primary physician participates as an active team member directing your care. Your physician must be a member of the professional staff at Columbia Memorial Hospital or Providence Seaside Hospital.
History of “The Hospice House”
Betty Cleator was a Lower Columbia Hospice patient in 1997. Her friend and companion, Freddie, had also been a Hospice patient in 1987. Betty left her estate to Lower Columbia Hospice. The executor of the estate approached Columbia Memorial Hospital with the idea of using the house to help other hospice patients who needed caregivers. The idea was approved, and work to remodel the house began in March 1998. Numerous donations of time, labor, equipment and money provided by business, community groups, and individuals made it possible to open the facility as of February 1999.
What is provided:
- Room with own bed and supply area.
- Bath 2-3 times a week
- Daily personal care
- Three meals plus snacks daily.
- Medications managed and supervised.
- 24-hour supervised living.
- Linens changed weekly or as needed.
- Laundry of personal clothing.
- Care to terminally ill via hospice plan of care.
- Smoke free environment.
What is not provided:
- Personal clothing and toiletries.
- Money Management
- Incontinence supplies
- Ostomy supplies
- Medications not related to hospice care.
- Hair dressing/Barbering
- Transportation
- Tobacco
How to qualify for the Hospice House?
You must be a current patient of Lower Columbia Hospice. You are required to provide payment for residential care via private payment or approved medicaid contract.
