Swing Bed Care
Not every patient is ready to go home from the hospital right away. If you are recovering from an illness or a medical treatment, Columbia Memorial Hospital's swing bed care program offers the continued care you will need to ensure a smooth transition from your time in the hospital to the next stage of your recovery, whether that means returning home or going to another care facility.
In our swing bed care program, we will focus on achieving your individual health goals until you are fully ready to heal at home.
How does swing bed care work?
Swing bed care is a Medicare-certified program that is available to most patients who need skilled inpatient care after a medical treatment or surgery. If you qualify for swing bed care and beds are available, you will move from our acute care unit to a swing bed unit. You will stay there during your entire recovery. During that time, we will bring care to you — you won't have to move to other units or schedule outpatient appointments.
We can provide a wide range of services to help you heal. Our care team will work with you to create an individualized plan to meet your goals. For example, that might include:
- IV medications.
- Monitoring vital signs.
- Physical therapy.
- Occupational therapy.
- Speech therapy.
- Wound care.
Who looks after swing bed patients?
As a Planetree-certified hospital, listening to your needs and preferences as well as meeting them is our top priority. Because we want you to regain your independence as quickly as possible, you will be cared for by the same team of medical providers from day one:
- A CMH hospitalist or your primary care physician will provide overall guidance and care.
- Our nursing team will be available 24/7 to give you medications and monitor your recovery.
- A care manager will help you prepare for going home and can talk with health plans and other agencies on your behalf.
- Rehabilitation specialists will provide therapy based on your specific needs.
- A social worker will meet with you to identify how they might be able to support you during your transitional care.
We also know that rest is important when you're recovering. That's why we minimize interruptions, such as night-time vital sign checks. You can recuperate in comfort while still having quick access to care when you need it.
When will I be ready to go home?
How long you stay in swing bed care depends on your health needs. It's typically anywhere from two days to two weeks. Determining when you are ready is a collaborative process. Your care team will meet regularly with you and your loved ones to discuss your progress toward your goals. And when the time comes, we'll help you make a smooth transition home.
