Community Health Needs Assessment
Helping to achieve a healthier community
It is with great pleasure that we present the results of our 2025 Community Health Needs Assessment (CHNA) (versión en español: 2025 Evaluación de las Necesidades de Salud de la Comunidad). This assessment reflects a robust and inclusive process that incorporated community surveys, public health data, listening sessions and interviews with a broad range of community members.
In 2025, CMH, Providence Seaside Hospital (PSH), and the Clatsop County Health Department partnered to complete a single, shared CHNA for our region. A central goal of this effort was to ensure that as many community voices as possible were represented. To support this, listening sessions were conducted in both English and Spanish, and community partners helped connect us with unhoused individuals to ensure their perspectives were included.
The result is a comprehensive and thoughtful assessment that accurately reflects the health needs, challenges and priorities of our community.
What is a Community Health Needs Assessment?
As a tax-exempt hospital, CMH is required by the Patient Protection and Affordable Care Act to conduct what is called a Community Health Needs Assessment (CHNA). This is a study that collects information to help us, and our community health partners, better understand the most common and challenging health issues in our region and work together on a plan that best addresses the findings and achieves the goal of becoming a healthier community.

This document informs our decisions when creating our strategic plan and community benefits plan. The CHNA is conducted not only to fulfill the requirement of the Patient Protection and Affordable Care Act of 2010 (H.R.3590) for nonprofit hospitals, but also in response to the CMH mission "We help people live their healthiest lives."
Turning information into action
CMH cares for our community inside our walls and out in the community. As part of the Community Health Improvement Plan (CHIP) available in May 2026, CMH will work to address key areas of identified need over the next three years.
