Community Sponsorship and Grant Program
CMH provides sponsorships and grants to community organizations and events on a bi-annual basis each year. A review committee made up of a Foundation board member, leadership team members and a patient representative will review requests twice annually.
Decisions on requests are made within a month of the deadline, with recipients notified shortly after.

Sponsorship requests
Sponsorship requests ask for funding of $500 or less. Typical organizations that are sponsored are: youth sports teams, schools, groups and clubs, and others. Sponsorship requests must be related to health and wellness.
To be approved for a sponsorship from CMH, applicants must fill out the sponsorship request form, including all required uploaded materials, by the following dates:
- Last day to submit for first review: March 31 each year
- Last day to submit for second review: September 30 each year
Grant requests
Grants are requests for funding for an organization or event. Any amount can be requested, but the review committee will make the final decision on grant amounts given. Examples of grant requests may include (but are not limited to):
- Community sports events (such as runs/walks) or exercise classes
- Cooking classes
- Volunteer events
- Equipment, such as an AED
- Educational talks about health
- Health screenings
- Health fairs
- Vaccine clinics
- First aid trainings
- Community baby showers
- Nonprofit support events
Grant requests must come from nonprofit or governmental organizations and must be related to health and wellness. They will be considered based on their relevance to an annual priority. The priority will be announced on this page each year. Priorities may include things like:
- Improving access to health care
- Enhancing community health and wellness
- Improving health education within the community
- Responding to a community need identified in the most recent Community Health Needs Assessment
- Addressing a social determinant of health
- Offering health care education
The 2026 priority for grant requests is health care-related requests.

Organizations that receive a grant will be asked to share a short report with CMH on how their event/program went, including number of attendees, information given/discussed, and any other relevant info. If a report is not submitted within two months of the event or program, the organization will not be eligible to receive future grants or sponsorships from CMH.

At this time, there are no reporting requirements for the organizations receiving sponsorships from CMH.

Yes, the grants may go toward helping fund an event.
The following are unlikely to be given a sponsorship or grant from CMH (please note, this is not an exhaustive list):
- Organizations, groups or events that do not serve Clatsop County (OR) or Pacific County (WA)
- Organizations that are not nonprofit or governmental
- Political organizations, for the purpose of furthering their mission
- Religious organizations, for the purpose of furthering their mission
- Requests from a single person or family
The requesting organization's completed W9 form is required, along with an official request for a sponsorship or grant on letterhead or other official organization correspondence.
Sponsorships and grants are awarded by the review committee within a month of the submission due date.
You will be notified by email of the committee's decision.
No, the review committee's decisions are final. If not selected, requesting organizations may re-submit their request for another review period.
Organizations may only receive one sponsorship or grant per calendar year.
Exceptions may be made on a case-by-case basis; for example, a sponsorship approved for a youth sports team and a grant given to the same nonprofit to support an annual event.
CMH offers sponsorships in amounts of $500 or less.
Any amount can be requested for a grant, but the review committee will make the final decision on grant amounts given.
Previous grant requests have included:
- $19,000 to purchase a LUCAS 3 chest compression device
- $2,000 to purchase an AED
- $1,000 to help fund a local high school booster club
- $500 to help fund a local 5K run/walk
- $500 to help fund a nonprofit event
