Aging Well Health Fair to be held in Seaside

Are you a senior with questions about community services, insurance and aging well in Clatsop County?
Join CMH for a free “Aging Well Health Fair” on Monday, September 22, from 1 to 4 p.m., at the Bob Chisholm Community Center in Seaside (1225 Ave A, Seaside, OR 97138).
The event will include health screenings, information on community services, demonstrations, information booths, door prizes, food and giveaways. More than 25 organizations, including CMH, Clatsop Care, Providence Seaside and Clatsop Community College will be there to serve you.
This event is sponsored by Connected Care. Connected Care is a program available at CMH that is focused on individuals age 55 and older who are medically frail and lacking support outside the health system. Through the program, they are provided with information and education, are connected with community services, and their health care is coordinated with their primary care provider.
