Clinic named a 5-Star Patient-Centered Primary Care Home by the OHA

Last week, the CMH-OHSU Health Primary Care Clinic – Astoria was surveyed by the Oregon Health Authority to verify its application to become a Five-Star Patient-Centered Primary Care Home.

The survey was successful, and Astoria Primary Care is now the first of the CMH-OHSU Health clinics to receive this level of designation. Prior to the survey, the clinic was recognized as a Tier 4 clinic. The recent additions of clinical pharmacy and psychiatric nurse practitioner visits allowed the clinic to be designated as the higher, Five-Star level of recognition.

According to the OHA’s website, “A Patient-Centered Primary Care Home is a health care clinic that has been recognized for their commitment to patient-centered care. In a Patient-Centered Primary Care Home, you’re the most important part of your care.”

This week’s survey was preempted by a lengthy application process completed by Jeanette Schacher, Medical Group director, and Kerrie McKey, clinic manager, in May. The surveyors interviewed 15 caregivers, as well as a panel of patients, to determine that designation was achieved.
