Improving the patient experience with Pre-Registration Intake forms

Dear patients of CMH and the CMH-OHSU Health Medical Group,

On Monday, August 14, 2023, we will be launching the second phase of our new appointment reminder system, Phreesia. You may already have been receiving appointment reminders through the system since we launched it in April.

The system allows us to send appointment confirmation texts, emails and phone calls, as well as provides chat functionality and the ability to cancel/reschedule appointments. These texts and calls may come from an unknown number with an area code of either 833 or 844.

Over the next four weeks, in addition to the appointment reminders, patients will be asked to fill out or verify Pre-Registration Intake information. The Pre-Registration includes, but is not limited to, the following items:

• Primary Care Provider
• Responsible Party Information
• Emergency Contact Information
• Insurance Information
• Medicare Secondary Payer Questionnaire (if applicable)
• Co-pay
• Consent Forms
• Photographs (only if using cellphone)
  • Patient Photograph
  • Driver’s License
  • Insurance Card

In addition to the Pre-Registration information, patients scheduled for appointments on or after Monday, August 14, 2023, may receive a final automated reminder one hour prior to their appointment. This gives patients one final reminder and a link to the Pre-Registration information if they haven’t already filled it out.

Furthermore, over the next few weeks, we will also be sending out new patient questionnaires to our new patients. These will be electronic versions of the questionnaires to which you’re accustomed.
