Insurance Information

Columbia Memorial Hospital has contracts with most local commercial insurances, Medicare and Medicaid. To make the most of your health insurance benefits, be sure to let the registration clerk know your insurance coverage and have your card available at the time of your non-scheduled outpatient service. If you have a scheduled procedure, you must give us your most current insurance information at the time you are scheduling the procedure. However, you need to be aware of your coverage that you have chosen with your insurance plan and understand that you may be required to pay any co-payments, deductibles and/or patient portion of financial responsibility. You will be expected to pay any co-payment and/or deductible amount prior to services being rendered.

If your insurance company does not pay promptly, we will contact you to schedule payment arrangements.

If you are a self-pay patient, we will prepare a detailed Good Faith Estimate of your charges and discuss a flexible payment plan. We offer discounts for patients who arrange a "prompt payment," with various payment options available to you.

Before receiving non-emergency services, please check with your insurance company to ensure that:

  1. CMH is "In-Network" or contracted.
  2. You have any necessary prior-authorization for services.

Medicare

If you have Medicare, CMH will file your claim for you. If you also have a Medicare supplemental policy, we will bill it after receiving Medicare's payment. CMH is a Medicare-designated Critical Access Hospital. This means that you will receive two bills for your stay at CMH:

  • If you are admitted to the hospital after an emergency room visit.
  • If you are in observation status before your inpatient admission.

Medicare Advantage plans

Below is a list of the Medicare Advantage Plans that are contracted with Columbia Memorial Hospital:

If your plan is not listed, you are responsible for contacting your insurance provider directly to understand your out-of-network benefits, coverage details and potential out-of-pocket costs.

Getting health insurance

If you don't have health insurance, or you need to find a new health insurance provider, you have options. You can compare insurance plans and prices on HealthCare.gov. Best of all, you may qualify for financial help paying for the insurance.
